If you are interested in your child attending St Mark Catholic School, please complete the information below. Within a week, you'll be contacted by a School Representative with more information. Completing this form does not guarantee admittance to Saint Mark Catholic School.

Records Required For School Enrollment
- Copy of Birth Certificate
- Copy of Baptismal Certificate (Catholic)
- Kentucky Immunization Certificate (if out of state, the Madison County Health Department will transfer the information to State of Kentucky form)
- Eye Examination by Ophthalmologist or Optometrist by January 1st of year of enrollment (Pre-K and K)
- Varicella Vaccination (only if child has not had chicken pox)
- Physical Examination
